We do urge you to note down your name/details so that procedures can be followed and parties can be contacted where needed and appropriate.

This form will only be seen by the Media Convenor and the Welfare Officer, more serious issues will be passed on to the Executive committee where appropriate and any broader issues will be passed to the entire committee unless requested against in this form.
